Initial Comments:
An off site follow up survey was conducted on 8/17/2021, Spiritrust Lutheran Home Care and Hospice, corrected the deficiencies and was found in compliance with the requirements of 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of an unannounced on-site hospice Medicare re-certification and state relicensure survey conducted between 6/2/2021-6/4/2021.
